由于颈椎长期劳损、骨质增生,或椎间盘脱出、韧带增厚,致使颈椎脊髓、神经根或椎动脉受压,出现一系列功能障碍的临床综合征——颈椎病,这是一种以退行性病理改变为基础的疾患。此病是中老年人的常见病和多发病,甚至一些青年人也患有此病。
针对此病,本文设计了一个颈椎椎间融合器,此椎间融合器植入人体颈椎后,能稳定脊柱,形成骨性融合,阻止椎间盘的退行性形变,一定程度上恢复椎间隙的高度。
本文通过软件mimics对颈椎进行重建,接着通过软件Solidworks进行模型建立,再利用软件Solidworks和 ANSYS接口导入进行有限元分析。
本文设计的颈椎椎间融合器经过脊柱重建后的总形变发生在施加载荷那一面的锯齿上,而最小的总形变发生在施加约束条件那一面的锯齿上,最大等效弹性应变和最大等效应力发生在右侧表面中间偏后处的位置,最小等效弹性应变和最小等效应力发生在下表面锯齿的尖端。
此颈椎椎间融合器的强度满足颈椎的生物力学特点,但是在设计时没有考虑到植骨窗和与椎间盘接触的面积之比,正确的设计应该是保证植骨窗的面积可使填入的骨量,但是也要保持适当的接触面积,如果接触面积过小就会导致沉降效应。还有设计时也没考虑到锯齿的高度,如果锯齿过高,同样也会发生沉降效应,从而导致颈椎的退行性形变,但是如果锯齿过低,会导致椎间融合器滑脱,这样就起不到恢复椎间高度的作用。
而选择的材质PEEK的椎间融合器也存在着很多局限性,PEEK材质椎间融合器不能被机体吸收,也存在着占位效应,对再次手术或临近节段手术影响较大,cage作为异物在体内长期存在,仍是潜在的排斥反映源,cage外形的设计,仍与正常颈椎间隙形态不符,不利于促进骨融合。
