Between May and July 2008, contract workers and Caterpillar employees working at Caterpillar facilities have experienced the following fatalities or severe injuries:
May 15th two contract workers fell from a man-lift, resulting in one fatality and one severe injury.
June 3rd a contract worker was crushed and fatally injured by a steel plate that was being transported down a ramp.
June 19th a Caterpillar employee was critically injured when struck in the head by a large piece of steel that the employee was maneuvering with a hoist and lifting fixture.
July 1st one contract worker was critically injured when the fully extended scissor lift on which he was working was struck by an overhead crane, throwing the worker to the ground.
Is there anything in this list that we don’t do at Perkins?
Good Conditions and Good Safety Systems do not stop accidents on their own …..
We need people to do the safe thing as well!
We must think about and choose safe behaviours that protect us and others from harm.
At the beginning of January you were all shown the video S.T.O.P a second for safety, which is a checklist for your personal safety
When you attend an unfamiliar area, carry out an unfamiliar task, or one in which your health and safety may be at risk, take a “second” of your time to consider the following :
Stop - Stop where you are and get into a position of safety before you expose yourself to a hazard
Think - Where you are? What are you doing? Are you prepared and able to carry out the work? What are your responsibilities?
Observe - Observe what is happening around you! What hazards are you exposed to?What is the risk? Is the risk acceptable?(yes or no)
Plan - Your safe route to your tasks. The safe completion of your task. Your safe way out.
We are currently performing very well
However, in our work environments we have the potential, right now, for serious injuries
It only takes a second for an incident to occur!
There has been an increase of reported incidents over the last few months of speeding and dangerous driving on site.
The speed limits on all sites are :
Inside 5mph
Outside 15mph
This will not be tolerated and therefore, could result in an investigation of the employee concerned and possible disciplinary action being taken

If you see an unsafe act or condition ….Fix it or report it ! If you don’t you will be condoning it !
